I confirm that no credit card and / or unsecured loan (including without limitation personal loan, tax loan and any loan with a revolving nature) under my name issued or provided by any financial institutions has been cancelled due to default in payment, and there is no current overdue payment exceeding one month in respect of my indebtedness (including credit card and any unsecured loans) with other financial institutions. I further confirm that no bankruptcy order has ever been made against me and I am neither in the process of petitioning for bankruptcy nor have any intentions to do so.

I, the undersigned, declare the above information and documents enclosed are true and authorise CMB Wing Lung Bank Limited ("the Bank") to disclose to, verify and exchange such information with and to obtain other credit information of myself from whatever sources the Bank may consider appropriate. I also acknowledge and agree that from time to time, all personal data relating to me ("the data") may be used and disclosed by the Bank for such purposes and to such persons in accordance with the Bank's policies on use and disclosure of personal data as set out in statements, circulars, notices or terms and conditions made available to customers. I further agree that the data and such other personal data and information relating to me may be used for conducting matching procedures (as defined in the Personal Data (Privacy) Ordinance) or be disclosed for marketing, credit checking and debt collection purposes.

I agree to be bound by the Terms and Conditions of CMB Wing Lung Bank RMB Credit Card Cardholder Agreement, which has been reproduced in the box below. A copy of which will be sent to me with the credit card upon approval of this application.

The annual fee will be waived for the first three years commencing from card issuance. I agree to pay the full annual fee of RMB550 for each Gold Card or RMB220 for each Classic Card after three years if I decide to keep the Card. I also acknowledge that interests for both unsettled credit purchase and cash advance are calculated at the annualised percentage rates (APR) of 20.05% p.a. and 21.14% p.a. respectively. The annual fee and finance charges are subject to the Bank's latest notice.

I also understand and acknowledge that if I give any incorrect or false information, I may be guilty of criminal offences related to deception and false information under the Laws of Hong Kong Special Administrative Region.

Important Notice to Customers relating to the Sharing of Consumer Credit Data

There are three options to submit your application:

  1. Press the ”Send” button below. Upon submission of application, please fax copies of the following documents (with application number marked on each page) to 2771 2482. Our colleague will contact you shortly after we receive your documents.
    - Your HKID card.
    - Bank account records/statements showing the past three months’ salary (including those pages showing the Bank name, account name and account number).
    - Latest residential proof, e.g. electricity bills/bank statement.
    - Other asset proof, e.g. fixed deposit advice, mortgage loan statement/repayment slip.
    - Business registration certificate and company tax assessment (if you are the sole proprietor of, or a partner in a business).

    Remark: Please be advised that the Bank is not responsible for any information being intercepted during data transmission if you choose this option.

  2. Mail the application form together with all required documents to Credit Card Centre, CMB Wing Lung Bank Ltd., P.O. Box 72569, Kowloon Central Post Office, Kowloon, Hong Kong.

  3. Submit in person the application form and all required documents to any Wing Lung Bank Branches.

Note: This application form and all supporting documents' copies are not returnable.

Your Opt-out Choice
Upon the termination of the account by full payment and on the condition that there has been, within 5 years immediately before account termination, no default in payment for a period in excess of 60 days on the account, you will have the right to instruct the credit provider to made a request to the credit reference agency to delete from its database any account data relating to the terminated account.
